COMPLYING WITH ORDER BY OWRD, 10" WELL LINER CASING WAS REMOVED AND 12" CASING WAS REMOVED. HOLE WAS REAMED TO 15" DIAMETER, NEW 12" CASING WITH PERFORATIONS FROM 118' TO 168' WAS INSTALLED. HOLE WAS GRAVEL PACKED FROM 168 BACK TO 115' WITH 3/8" PEA GRAVEL. CEMENT GROUT WAS PLACED FOR SEAL. ORIGINAL SEAL WASHED OUT DURING REAMING OPERATIONS. COMPLETE NEW SURFACE SEAL WAS INSTALLED FROM 115' BACK TO SURFACE.
