Reconstruction procedure: Perforated 8" casing from 60 ft to 413 ft. Oregon Water Resources ran camera to examine and approve perforations. Placed pea gravel from bottom of hole, 500 ft to 420ft. Installed casing from +2 to 420. Ran 2" grout shoe at 420. Pumped 121 sacks thru grout shoe, sealing from 126 to 420, method A. Returned 36 hrs later, drilled cement show out and cleaned hole to bottom 500 ft. Pumped 196 sacks cement between 6" and 8", filling to ground surface, 0 to 126. Method C.
Reconstruction procedure page 2: Over drilled the 8" casing with 12" casing down to 30 ft. Pressure grouted from 30 ft bask to land surface with 18 sacks cement, method c. Special standards granted 8/13/2010. Special standards granted 9/7/2010.
